Sample report forms and instructions for how to submit reports
Refer to your grant agreement for the requirements of your grant and which form to submit. The sample reports shared on this page are for reference only. Progress updates or reports should be submitted through akoyaGO, the Walder Foundation’s grants management system. Please do not send reports or updates by email unless otherwise directed by your Walder program team member or Grants Management.
PROGRESS UPDATE
A written or verbal progress update to include a summary of progress, achievements, and challenges. (You may have a phone meeting with your program officer instead of completing this form).

PROGRESS REPORT
A written report including a narrative summary of progress, achievements, and challenges and a financial report detailing the budgeted to actual expenses to date.

FINAL REPORT
A written report including a narrative summary of progress, achievements, and challenges and a financial report detailing the budgeted to actual expenses.
RESEARCH REPORTS
RESEARCH PROGRESS REPORT
A status report on how the project is progressing and any research findings obtained; grant expenditures to date; and justification for any proposed revisions in the project plan, timelines, milestones, and budget.

RESEARCH FINAL REPORT
A final report to provide 1) a candid assessment and report about any research findings obtained and lessons learned and 2) a financial report with budgeted and actual expenses.
HOW TO SUBMIT A REPORT
Important Note: If you have already created an account in our new Grants Management System, akoyaGO (which went live in April 2025), follow the instructions below. If you have not yet created an account, you must first do so following the instructions here.
To access your report, log into your account on akoyaGO:
Walder Foundation Grantee Portal
Navigate to the “In Progress” tab at the top. Find the report you need to complete, and click on the “View” box to the left to access and edit the report form.
Once you complete and submit, your submitted report can be found by navigating to the ”Submitted Tab”. Please note, if you are not the primary contact on the grant, you will see the form when in progress but not once it’s submitted.
